Add icd9 codes to PheWAS

This page explains that the file PheWAS_code_translation_v1_2.txt is not using the same phewas-icd9-mapping as we are. We need to use phemap.rda and pheinfo.rda.

Should we still pull our ICD9 strings from the v1.2 file, or look for them somewhere else?

• Convert phenolist.py -> phenolist/__init__.py

• Make phenolist/icd9-phewas.json which contains the information we need

• Make pheweb phenolist icd9-phewas. It checks for columns in pheno-list.json that look like phewas codes, and then augments the file if it finds any.
