When reading the README on https://github.com/MitMaro/git-interactive-rebase-tool, this avoids an unnecessary page navigation to https://github.com/MitMaro/git-interactive-rebase-tool/blob/master/README.md.

Also fixed a broken link (#usage -> #direct-usage).
