Paper Submissions Address field should default to United States

As an Administrator, in order to improve usability of form, I need to have country dropdown select form default to United States and not show - None - as an option. Also if a paper submission address is not required to submit the form make the street address, city, state and zip code fields not required. The Submission Address fieldset should be collapsed or expanded to show/hide the street address - NOT the selection of country.
