Requesters should be able to remove a previously uploaded document from the request form

If I upload a document to a request form and then change my mind about submitting it I should be able to remove it before submitting the request.

Expected Behavior

I should be able to remove a document from the Upload Documents field.

Actual Behavior

Once I upload the document I see no way to remove or delete it. I can only replace it with another document.

Steps to reproduce the behavior

1. Complete a request form and upload a document.
2. Try to remove the document from the Upload Additional Documentation field.
