Add issue template (reproduction is very important!)

It is always difficult to investigate an issue unless I can reproduce it on my end.

Unfortunately, issues submitted to this repository usually lack reproductions, and I almost always require reproducers to provide reproductions after the issue is opened.

I wonder if this situation could be improved by the use of an appropriate issue template.
