Dynamic Panel - A panel accidentally disappears when setting a default answer via the Set Default Answer dialog #6088

To reproduce the issue, follow these steps.

Add a Dynamic Panel; create an input field.
Set the initial panel count to 2.
Navigate to Data, activate the Set Default Answer dialog. You'll see there are two panels (the default number) appear in the dialog.
Supply a value for the first panel. The second panel accidentally disappears. Though you can actually add another panel and set its answer.
