Riemann slack notifications do not work with the new Slack apps.

Requests made to the slack webhook url https://[account].slack.com/services/hooks/incoming-webhook?token=[token] only work with legacy webhooks; any new webhook created with a slack app returns a 404, with the body no_active_hooks
