Found a strange issue and spent all day thinking it was a code issue.
Using Piper wrapped in PiperSharp for full disclosure.
On a laptop with fxsound installed, i was coming across sound audio skipping or dropping initial parts of the soundclip.
Debugging showed it was an actually issue with the generated wav files, ie in the piper service itself.
It appears that fxsound is somehow incompatible with piper - suggest either a fix or listing it as an incompatibility
