When I click on a parent deck to study all subdecks, the first card doesn't show the subdeck in the title, but all the rest do show the subdeck.

Yes, i think that is true.
IIRC that would need some medium-sized rework, and i admit i’m not really interested in spending much time on this.
I’m sorry, but please don’t expect a fix any time soon.

If you edit a card to a different deck while studying, on returning, Anki often pushes that card to a new position. The title bar does not update appropriately in that case. Probably related to this issue.
