Automatically create GitHub issue when problems are detected #1

When the workflow runs, if a problem is detected:

Detect if there already is an open issue that reports this problem.
If so, do nothing. If not, open an issue and ping core dev(s).
In any case, embed a link to the related issue in the service card.

I would refrain from automatically closing issues when the workflow no longer detects issues, but perhaps a comment could be posted.
