Bug in slug display when entries displayed as controller.

Hello,

Just found a bug that make you can't edit the slug of a related entry displayed as a controller.

Here is to reproduce :

            titleCreateForm: Nouvelle caractéristique

When you create a Boutique\TypesProduits item, and then add some Boutique\Caracteristiques to it, the published green button that appears on the top right corner of the popup is empty, so you can't change slug.

This was caused by $formModel not being passed to the header controls partial when entries are displayed in controller mode (relation popup).

A fix for this has been added in v4.0.20. Hopefully it helps
