Something odd with file registration

I'm not sure what kind of bug this is (although concurrency would be a first guess) or in where the fault lies, but something is signalling failure for the database queries, which seems wrong.It

(It seems I had multiple concurrent runs, uploading the same empty file, which may be less than useful, but I still don't like that pq error message.)
