[Dash] Documents page doesn't filter/indicate entries' source

Problem

In the example below, the first entry was uploaded by the customer:

There's no way to tell which record was downloaded from HIEs/external servers and which ones were uploaded by the customer.

Suggested solution

Add a source filter so users can choose whether to list documents uploaded by them or downloaded from the HIEs/networks.

By default, the filter would be set to downloaded documents.

If the user selects Both or no filter for source, then all records would be displayed and a column would be added indicating whether it was uploaded or downloaded.
