An already existing ToC does not seem get refreshed on Update. Need to test again.

The current implementation is at best a hack/kludge. It might work for headers that are added / appended at the end of the file.

The predictable way of generating an updated ToC is to completely remove it. Commit a change, and then come back and refresh it with a new ToC.
