Adding hyperlinks to PDFs

I have a single-column book of 350 pages (with some images) and wish to annotate words/phrases in the text by linking to an online dictionary of terms. Are there tools already in PDFPlumber that allow this?

I assume it doesn't need to reformat the page and could give a Wikipedia-like appearance of blue words, maybe underlined, maybe with tooltip titles for hover display. Word wrap will be a problem but I'm happy to forgo that initially.
