Replace { fd: 0 } with process.stdin, which should have better compatibility with Windows. Partial fix for #18

On windows (especially with Microsoft IIS) the FastCGI socket is still passed as the stdin handle, but the handle is not 0 (like on POSIX systems).
It can be obtained by a call to
GetStdHandle(STD_INPUT_HANDLE)
fromkernel32.dll
