Design: Let's build a contact form

Story

As a user, I want to contact the .gov team through an accessible web form.

Acceptance Criteria

• There is a contact form on https://get.gov/contact/ that is not a Microsoft or Google form
• The form is accessible
• The form includes the same fields as on the existing Microsoft form that is embedded on the contact page

Additional Context

The current Microsoft form is not fully accessible. A user can't tab through all of the form fields.

We get a lot of benefit from using the canned Microsoft form (particularly around Power Automate). Still, this is a good idea for the future.

needs refinement, I’m unclear the separation we want here between dev and design.

@abroddrick we've put this in the backlog because it's a good idea, but slotted it low in immediate priority for now. We'll refine it as it increases in priority.

The thinking was to not create two separate issues (design and dev) now, though we'd likely do that later. Instead, we labeled the one issue with both tags. Thoughts on that approach?
