Allow the .slideout-panel container to be a form

We're putting forms in slideout panels most of the time, but semantically, they're not a form and can't use built-in browser validation features. We should allow the div.slideout-panel to instead be a form with a prop, so we can have the form surround both the content of the panel and the toolbar.
