Consolidated After Action Report for all outages during Fall 22 Semester

Summary

We had a series of outages across all hubs which affected instruction during Fall 22. Like any other outage in the past, we should create a detailed After Action Report (AAR) and document the steps taken to resolve the source of these issues. List of outages encountered during FA 22 is detailed here.

Creating this issue to track the completion of these two activities.
