This issue does not happen all the time. Unsurprisingly it seems to be related to the slope of the terrain.
I was able to reproduce the issue fairly consistently by not using abilities at all to defend myself. If it helps here are a couple locations the dismount caused me to clip underground partially and fall all the way under the map once i tried to move:

.go quest starter 12308- Take the quest and follow through
- Make sure to let the horse die to get dismounted, preferably on a slope

Current Behaviour
Quest works fine unless the horse dies at which point you will fall through the map or at least I did. (https://www.wowhead.com/wotlk/quest=12308/escape-from-silverbrook)
Expected Blizzlike Behaviour
When the horse dies you should just be dismounted and forced to fight whatever mobs you had on you at the time of death.
