Add eligability questionaire

We'd like to add a simple questionnaire to the website to quickly rule people out that cannot expunge. Questions will be along the lines of: "How long ago was the crime? Are you currently convicted?"

Giving yes/no answers is not ideal - it invites liability and we cannot guarantee the answer. Instead, we should use the questions to provide educational information which people can use to base their judgement on.
