[DESIGN] Mock-up forms platform/index integration

Overview

• As an agency forms manager, I need to track all of our agencies forms, including where they are being accessed and if they are compliant with PRA.
• As a DOJ forms manager, I need to understand how our forms are performing, so that we can address any problems and/or have data to report on xx amount of submissions, etc.
• As a DOJ forms builder, I need to digitize a form, so that more fillers can access it.
• As a DOJ employee, I need to find a form, so that I have context to support fillers and/or can share the link with fillers.

Context

• Figjam with site maps, user stories and requirements, and mock-ups
• We want to explore what the Forms Index looks like as a feature of the Forms Platform
