survtab_fpa: the usual three time scales

survtab_fpa Would be a simple wrapper around survtab that asks for times of birth, entry, and exit as well as the status variable (somehow) and forms the appropriate Lexis object.

Could only ask for age and entry and use Surv() to pass follow-up time and status...
