Describe the bug
The existing form to submit a new UIPA request looks something like this:

With it having an existing form delimiter line. This is the source of confusion for many a form submitter, as they often do not follow the instructions.

Existing Important Notes that are often ignored:

Do not delete the [ALL CAPS text](https://uipa.org/help/faq/#10.5)!
Write your request in simple, precise language.
Ask for specific documents or information.
Keep it focused and concise.
This site is public. Everything you type and any response will be [published](https://uipa.org/help/faq/#7).
Do not include personal information (yours or anyone else's) in your request.
Do not ask for [personal information](https://uipa.org/help/faq/#11) (yours or anyone else's).
Avoid asking for [more than you need](https://uipa.org/help/faq/#12).
If appropriate, include a request for [public interest waiver](https://uipa.org/help/faq/#26).

Additional context

Note: Apparently there is something weird going on with the word Mahalo as well, that should be looked into also

I think it would clearer to have a separate input textarea to capture the reason for requesting a waiver. It would also make it easier to create the request that will be emailed to the desired public agency.

I did a tiny bit of work on this back in 2017. At that time, I thought it was weird but the goal was to fix some bug and get to production. So, now would be a good time to rethink things.
