Describe the bug

When initially creating a UIPA request, the following questions are presented:

Answering the "What kind of information do you want?" question with "a matter that involves you personally (personal records request)" gives us this:

This behavior is correct, stated in the given message. However, there is no way to backtrack from this answer and re-submit the form with the other option without refreshing the page entirely.

Expected behavior
Instead of having the user submit the form to see the given message, perhaps we highlight a message when the option is first selected instead. However, we could just allow backtracking from this given message in some other way as well.

It would be better to say to contact the public agency directly for information about personal records and not even have the step of choosing the kind of information being requested.
